The Walton Community Health Center located in DeFuniak Springs, Florida is seeking a mental health counselor (LCSW or LMHC) to provide integrated mental health services within a primary care outpatient clinic setting. The Walton Community Health Center is a certified Patient Centered Medical Home which provides comprehensive care to patients of all ages and types. This is a full time position.
The successful candidate will provide direct care to patients of the health center, coordinate mental health referral for patients needing expertise beyond the capacity of the health center, and serve as the mental health consultant to the primary care team in the health center. Special focus on substance use disorder and chronic pain treatment desired. Experience and proficiency with electronic medical record system and with telehealth desired.
Duties & Responsibilities:
- Access, analyze, diagnose, using DSM-IV, and treat those seeking outpatient psychotherapy.
- Complete clinical assessment for each patient in accordance with department policy.
- Document progress/data in medical record utilizing objective, measurable methods.
- Consult with clinical team; participates in clinical team meeting for treatment management.
- Actively participates in quality improvement program plan.
- Attends and participates in daily QI huddles and monthly QI meetings.
- Readily accepts assignments to QI subcommittees.
- Coordinates with Medical and Nursing Director in developing and updating clinical protocols.
- Participates in peer review as outlined in QI plan.
- Provides crisis intervention as needed.
- Explore local resources and meet with alternative service delivery providers to promote client access to health care through community agency collaboration.
- Provide linkage with appropriate community resources by maintaining knowledge of other systems, making referrals, and identifying current needs
- Maintains open communication with Administration to improve efficacy and productivity in clinic operations.
- Collaborate with peer group to ensure schedules utilized for maximum productivity.
- Develop, plan and coordinate behavioral health Medicaid programs and policies.
- Assist in research and grant writing.
- Prepare reports accordingly.
- Cultivate and maintain professional affiliations with colleagues, other community, and state agencies.
- Represent the CHD at local meeting, conferences, and workshops regarding assigned areas of responsibility.
- Serves as field instructor for students performing educational social work internships.
- Perform other related duties.
- Ensure accurate and timely submission of Employee Leave and Attendance Records, Employee Activity Records (EARs) and travel vouchers.
Hours: 8am to 5pm Monday thru Friday (40 hours per week with some possibility for flexibility during hours clinic is open)
No usual night or weekend hours. May be required to work other hours during emergencies.
