Harborview Medical Center has an outstanding opportunities for 2 full-time, day shift, MENTAL HEALTH PRACTITIONERS (CASE MANAGER). Under general supervision, the Mental Health Case Manager provides case management for assigned patients with serious and persistent mental illness. This position on the Recovery Case management teams provides engagement, case-management, outreach and linkage as well as counseling services. The Mental Health Case Manager is responsible for implementing the goals of the program, which are to promote community stability of patients by engagement in mental health services and treatment planning as well as assistance in securing and maintaining housing, benefits and culturally relevant treatment to meet their needs, and to assist patients in developing meaningful and productive activities. The Mental Health Case Manager will be expected to be able to work with a wide variety of patients.
